KTC Enrollment Transfer Form

This form is used to request a transfer of your enrollment into the KTC system. By submitting this form, you authorize placement within a designated enrollment structure and acknowledge that applicable charges will be processed on the specified billing date.

To preserve your current team structure, all required information must be submitted accurately and before the designated cutoff date. While we will make every reasonable effort to honor your existing genealogical placement, submissions received after the cutoff date or with incomplete information will not be guaranteed placement within the same structure or order.

Placement will be processed based on the timing of completed submissions and system requirements at the time of enrollment.
